- Vision Service Plan: No ID card with this Plan

This discount is available to BlueCross participants as well as other family members, even if they are not covered by BlueCross. Verify Student Status for Dependent Children by Aug. 31

BlueCross BlueShield of Illinois is requesting certification of student status. If you have a dependent child between the ages of 19-25, you will receive a Student Certification Form. You must complete the form to document full-time student status for the fall semester.
Please complete the Student Certification Form and return directly to BCBS in the return envelope provided. If the Student Certification Form is not completed and returned to BCBS, your dependent’s coverage will be canceled effective Aug. 31.

2008 FSA Claim Deadline is March 31

If you participate in a tax advantage account, such as a health care FSA, limited purpose health FSA and a dependent care FSA, claims incurred in 2008 need to be submitted to Bank of America by March 31, 2009, to be eligible for reimbursement. Any money left in the account after March 31 will be forfeited.

Confirm Your Benefits Election for 2009

We encourage you to go to Ceridian Self-Service to review your Jan. 15 earnings statement and your benefits summary to confirm the benefits you selected are correct for 2009. If your benefits elections are not correct, you have until Jan. 31 to make changes. To request a correction, send an e-mail to benefits@farmcreditfoundations.com. You must notify Foundations of any corrections in writing. We cannot accept any corrections to your benefits after Jan. 31, 2009, without a qualifying status change.
