Ready to make your elections?
- Complete your Online Enrollment Form
- In order to confirm your enrollment, benefit forms submitted without the electronic signature will not be processed.
After you have enrolled, you will receive an e-mail notification confirming your elections.
- Review your benefit summary on the e-mail. This information also is available on Ceridian Self-Service.
- Review your pay advice to confirm your benefit deductions.
- Report any discrepancies to your benefits administrator.
Defined Contribution/401(k) Plan
- After you have received your first system generated paycheck, Visit mylife.newyorklife.com or call New York Life at 1-800-294-3575 to make your 401(k) contribution and investment elections.
- If you do not make an election to participate in the 401(k) plan within 45 days of your hire date, we will automatically enroll you in the plan at a 3 percent contribution rate.
- Or include information regarding the funds, so they can review to make their elections once they have received their first paycheck
Group Universal Life Insurance for You and/or Your Spouse
- After you have received your first system generated paycheck, You have access to elect Group Universal Life coverage online at www.LifeBenefits.com.
- As a new employee, you can elect coverage at 1x your total compensation without medical review. You may also elect coverage for your spouse (up to $50,000) and your children without medical review.
- Your initial User ID and Password for enrollment will be:
User ID: FC plus your six digit Ceridian Clock Number (This will be provided by your Farm Credit HR Respresentative
Password: Your date of birth plus the last four digits of your Social Security number (e.g., MMDDYYYY####)
