Welcome to the Farm Credit Foundations Benefits Plan!
As a new employee, it is important you have access to the information needed to make an informed and confident decision about the benefits package you select. We have created a variety of tools and resources that we encourage you to use as you make this decision.
New Employee Enrollment Period
Your New Employee enrollment period is the first 45 days of hire. Benefits will be effective the 1st or 16th day of the month on or following enrollment. If you do not enroll within 45 days of hire, you will automatically be enrolled in the employer-provided benefits (Basic Employee Term Life and Accidental Death and Dismemberment, Business Travel Accident, Long-Term Disability Insurance and the 401(k) program). Once enrolled, your election is irrevocable unless a qualifying change of status occurs or you must wait until the next annual enrollment period to make changes.

- Make Your Benefit Elections Online
- As a new employee, you have 45 days from your date of hire to enroll in your benefits.
- Benefits will be effective the 1st or 16th day of the month on or following enrollment.
- Complete the Benefits Enrollment Form.
- Benefit forms submitted without the electronic signature will not be processed.
- Review Benefit Enrollment Confirmation E-mail
- You will receive an e-mail notification to confirm your elections.
This information also is available on Ceridian Self-Service (how to access). - Review this e-mail and report any discrepancies to your Benefits Administrator.
After You Receive Your First System-Generated Paycheck, Continue to Steps 4 and 5.
- Enroll in the Defined Contribution/401(k) Plan
- You will receive separate instructions for using www.mylife.newyorklife.com and enrollment directly from New York Life.
- After you have received your first system generated paycheck, you are able to go to www.mylife.newyorklife.com and access the tools and resources available and enroll in the Farm Credit Foundations 401(k) Plan.
- We offer you a dollar-for-dollar match of up to 6 percent of your total compensation with our 401(k) plan, which is administered by New York Life. Learn more about the plan and how to enroll.
- Enroll in Life Insurance for You and Your Family
- After you have received your first system generated paycheck, you have access Life Insurance coverage options through Minnesota Life. Learn more about the coverage and how to enroll by viewing the Group Life Insurance Plan Document.
- Visit www.lifebenefits.com and make your elections, review, make changes or update beneficiaries.
- For new users your ID and initial password are:
User ID: FC + your six digit Ceridian Clock Number (e.g.; FC123456)
Note: Your Ceridian Clock Number is provided to you by your Farm Credit HR Representative.
Initial Password: Your date of birth + the last four digits of your social security number (e.g.; mmddyyyy####)
