How to Make Changes to Your Benefits Plan
- Complete the Status Change Online Form.
- Once your benefit elections have been submitted and reviewed, you will receive an e-mail notification.
- Be sure to review your benefit summary, which is available on Ceridian Self-Service.
- Review your pay advice to confirm your benefit deductions.

You may need to update or change some personal information by logging on to Self-Service.
- Marital status (updating marital status does not add or remove your spouse from your health and welfare plan; you must complete a status change form to add or remove a dependent(s) from your benefit plans)
- Dependent information (adding or removing a dependent in Self-Service does not add or remove your dependent from your health and welfare plan; you must complete a status change form to add or remove dependent(s) from your benefit plans)
- Address/phone change
- Name change (requires updated Social Security card)
- Update emergency contact information
