Divorce

Getting divorced is a qualifying event that allows you to make changes to your benefits plan. 

 

How to Make Changes to Your Benefits Plan

  1. Complete the Status Change Online Form.
  2. Once your benefit elections have been submitted and reviewed, you will receive an e-mail notification.
  3. Be sure to review your benefit summary, which is available on Ceridian Self-Service.
  4. Review your pay advice to confirm your benefit deductions.

  • Life Insurance

    You may also want to consider updating your beneficiary designations.
    To elect coverage and designate beneficiaries log onto www.LifeBenefits.com.
  • Retirement Plan

    Want to change your 401(k) deferral percentage or update your 401(k) beneficiary designations?
    Go to mylife.newyorklife.com.
  • Payroll/Taxes

    Need to make changes to your payroll, direct deposit information, and tax situation?  Go to Ceridian Self-Service to make changes to your tax withholdings or direct deposit information.
  • Personal Information

    You may need to update or change some personal information by logging on to Self-Service.

    • Marital status (updating marital status does not add or remove your spouse from your health and welfare plan; you must complete a status change form to add or remove a dependent(s)  from your benefit plans)
    • Dependent information (adding or removing a dependent in Self-Service does not add or remove your dependent from your health and welfare plan; you must complete a status change form to add or remove dependent(s) from your benefit plans)
    • Address/phone change
    • Name change (requires updated Social Security card)
    • Update emergency contact information
