Filing a Claim
Participating Provider
- Participating provider will work directly with VSP
- No claim form necessary – benefits will be applied at point of sale
Going Out of Network?
- VSP pays a service allowance for non-participating providers.
- You will need to pay the entire bill at the time of service and submit a claim for reimbursement to VSP.
- Out-of-network claims must be submitted to VSP within six months from the date of service. Go to www.vsp.com to file your claim.
VSP Non Provider Claim Form is used if you see a non Vision Service Plan provider and need to submit a paper claim.
