Filing a Claim
If you fail to present your ID card to purchase a prescription at a Network Pharmacy or if you use a Non-Network Retail Pharmacy, you may be required to pay the full cost of the prescription and then file a Claim with Caremark for reimbursement of eligible expenses within 12 months of purchase.
