Consumer Choice PPO Plan

The Consumer Choice PPO Plan offers flexibility to see any health care provider you choose, however when you go to an in-network BlueCross BlueShield provider you are going to receive services at a negotiated discounted fee.

 

The Consumer Choice PPO Plan is a qualified high deductible health plan (HDHP), which entitles you to a health savings account (HSA). Be sure to review the HSA section for more information!

 

The Consumer Choice PPO Plan has a $2,700 deductible for employee coverage or $5,450 deductible for all other tiers. One family member or a combination of family members can satisfy the family deductible and the full family deductible must be met before post deductible benefits are paid.

 

Locate a Network Provider

BCBSIL has a large network of providers.  You can find a list of participating providers by logging on to or enrolling in your account at www.bcbsil.com/foundations, or by calling BCBSIL at 1-866-563-8366.  Also, call you doctor and ask, “Do you accept BlueCross BlueShield?”

 

 

How do I Make Changes to My Plan?

Become a BCBSIL Blue Access Member

We encourage you to log in and open your personal Blue Access account at http://www.bcbsil.com/foundations.  To open your account, you will need to create a user name and password.  When you enroll in Blue Access, you can:

  • Search for doctors and hospital in your area
  • Check to see if a claim has been paid
  • Check on the cost of a particular medical procedure
  • Sign up for alerts about your claim activity
  • Print a temporary ID card
  • View up to 18 months of claim payment details
  • Enroll in a program to lose weight or stop smoking
  • Learn more about a specific condition or disease
  • Get other important medical information

 

 

Preferred Provider Organization (PPO)
An organization in which networks are created with Preferred Providers (Medical/ Dental professionals who have contracts with the organization to provide services at a discounted rate). Generally benefits will be greater and costs will be lower if the participant uses an in-network provider.
