Am I required to use a PPO provider?
The Delta Dental PPO Plan offers a two-tier nationwide network of providers. You can also choose an out-of-network provider. The two Delta networks are:
- Delta Dental Premier Network
- Delta Dental PPO Network

How do I Make Changes to My Plan?
Changes to your pre-tax benefit elections – outside of your original eligibility period or annual enrollment – are only allowed in the event of a qualifying change in status. Changes to your benefit elections due to a life-changing event must be made within 31 days of the event. Learn More ...
